Qualifying Life Events - Employee Instructions
How does an employee submit a life event?
Proof of Your Life Event
You will need proof of your life event, depending on what your change is. Scan the documentation and save it as a file. You will be required to attach this file before you are allowed to submit your life event change.
A few examples of required documentation:
- Termination Letter from Employer
- Birth Certificate
- Court Order
- Marriage License
1. Getting to the website
Open your Internet browser and enter:

2. When the EasyAppsOnline website opens, enter your Username and Password and click the GO button. If you don’t remember your login information, click the FORGOT LOGIN INFO button.
Click the appropriate button to find the information you need: Retrieve My Username, Retrieve My Password or Reset My Security Question. Enter your Email Address and click Email Username. If the system doesn’t recognize your Email Address, click the Click Here link at the top of the screen for an advanced account lookup tool.
3. Click on Submit a Life Event to start the process.
4. Select the reason for your life event from the drop-down menu
NOTE: Next to the list of reasons, you can click the View Life Event Tutorial button for assistance.
5. Click Submit this Life Event.
6. Enter the date of the event and click Continue to Next Step...
7. Follow the instructions in the next step – this will vary, depending on the reason you chose
You may need to Add, Remove, or Modify Dependents and/or Coverage.
8. Adding a New Dependent
Fill in your dependent information and Click SAVE.
9. Changing/Adding/Removing Coverage
Click the MODIFY MY COVERAGE link and check or uncheck the boxes for all applicable members (yourself and/or your dependents).
10. Click Continue to Next Step...
11. Click +Add Supporting Documents (2mb limit per file)
Navigate to the document you saved documents that provide proof of your life event change and attach it. Click the UPLOAD button to attach the file to your life event. You can attach mutliple documents if needed.
12. Sign your Name and click I approve this Life Event
You will be emailed when your life event change has been approved, not approved or if you need to provide additional documentation. Click the Back to Main Menu link to log out.
