How do I map or set up the EOI questions on carrier applications?
Part I: EOI Application Mapping on Final Plans – Voluntary Life Plans
Make sure the Voluntary Life plan is set to show the applicable carrier for this plan. In this example, we will be using Guardian as the carrier. The carrier has probably already been selected, since this is a required field when you first create the plan. You will also need to make sure you have an Employee GI Amount indicated in the plan setup (you can also set one for Spouse, if applicable) and also set the Action to Allow but flag election census. If you made any changes on this screen, be sure to click the Update Deduction Period-Plan Name button to save the updates:
Find your Voluntary Life plan in your list of plans for the group and look for the yellow bar where you map the plan on the carrier application to your final plan. If applicable, map the 1st yellow bar to the main carrier form that you are using for enrollment, but this is not required to make the new EOI mapping work. Be sure to choose the applicable plan type/name from the list:
When the system sees that the carrier you’ve chosen in the plan setup in Step 1 has EOI forms available in our library, it will automatically take you to the EOI Question Mapping screen. On this screen, select exactly which type, or set, of EOI questions you need to ask users for your plan. Make sure the name of this set of EOI questions matches with the setup you’re trying to do. In this example, we are asking EOI questions to any user who selects a Voluntary Life Amount Over the GI Amount, which matches what we see in this drop-down list. Click the Save button when you are finished.
NOTE: If you DON’T need the EOI questions for this plan (i.e. Basic Life plan), or the EOI questions are being asked on a different form, click Skip EOI Question Assignment. You will have the opportunity to map a different application for the EOI questions in the next step.
Go back to your Voluntary Life plan in your list of plans and look for the 2nd yellow bar for mapping the EOI application. You MUST do the EOI Question Mapping a second time from the 2nd yellow bar.The forms shown in the drop down list will be any EOI form that we have in our library for the applicable carrier, specific to the plan type you’re using. In this example, we’re using Voluntary Life on a Guardian plan, so the system shows any application we have programmed for this carrier that also is set up to ask EOI questions for any Voluntary Life product. There might be a few different types of Voluntary Life EOI applications available for this carrier, so be sure to select the correct form/option from the list. The form with the EOI questions on it might be identical to the one you mapped your plan to, or it might be a different form:
NOTE: Once you select the form from the list, EasyAppsOnline will automatically attach this EOI form to your group in the application listing, if it wasn’t already attached.
IMPORTANT: If you don’t see the EOI form or option you need for your plan, contact us right away so we can work with you on what you need for your group.
On the next screen, select the same type, or set, of EOI questions you selected previously in Step 3, IF you did any EOI Question Mapping in Step 3. Again, like before, in this example, we are asking EOI questions to any user who selects a Voluntary Life Amount Over the GI Amount, which matches what we see in this drop-down list:
Click the SAVE button to save these settings. Your EOI form mapping is now complete for Voluntary Life.
Part II: EOI Application Mapping on Final Plans – “Other” Plan Types
NOTE: The Other Plan setup type is usually used for plans like critical illness, hospital indemnity, etc. You can use this function with both Other (Composite) and Other (Age Based) plan types.

Make sure the Other plan is set to show the applicable carrier for this plan. In this example, we will be using Transamerica as the carrier. The carrier has probably already been selected, since this is a required field when you first create the plan. If you made any changes on this screen, be sure to click the Update Deduction Period-Plan Name button to save the updates:

Find your Other plan in your list of plans for the group and look for the yellow bar where you map the plan on the carrier application to your final plan. Map your plan to the applicable plan on the application – this will start the process for setting up the EOI questions:

You will immediately go to the EOI Question Mapping screen. Select exactly which type, or set, of EOI questions you need to ask users for this plan. Make sure the name of this set of EOI questions matches with the setup you’re trying to do. In this example, we are asking EOI questions to any user who enrolls in the Hospital Indemnity coverage, which matches what we see in this drop-down list:

Click the green SAVE button to save these settings. Your EOI form mapping is now complete.
NOTE: On “Other” types of plans, you only need to map the EOI Questions once; whereas on Voluntary Life plans, you have to go into the EOI Question mapping screen twice.
